Caracterizar a demanda de atendimento psicológico e compreender a experiência dos profissionais que atuaram na pandemia do COVID-19, em uma plataforma de atendimento psicológico on-line. Realizou-se uma análise documental e entrevista semiestruturada com 20 psicólogos que atuaram no mínimo 6 meses na plataforma entre mar/2020 à dez/2022, em que foram atendidos um total 15.963 pacientes. Dentre as demandas, prevaleceram ideação suicida com planejamento, ansiedade e depressão. O perfil prevalente dos pacientes atendidos na pandemia corresponde à literatura, em períodos não pandêmicos, (...) bem como a prevalência de sintomas ansiosos e depressivos. Porém, destacou o surgimento do alto número de pacientes com ideação suicida, o que sugere um agravamento dos transtornos mentais, despertado pela pandemia, além de uma dificuldade comum aos entrevistados, relacionada a clareza dos critérios clínicos utilizados para determinação da gravidade dos casos. Os psicólogos apontaram dificuldades no manejo destes casos graves, intensificados pelo impacto emocional pandêmico também sentido por eles, junto ao despreparo para sua atuação online, as percebendo como limitações. Apesar do despreparo, a alta demanda incitou os profissionais aderirem as especificidades da modalidade. Estes relataram ter buscado aporte teórico e apoio emocional para manterem os recursos internos, cognitivos e emocionais, necessários ao setting terapêutico. Pode-se concluir que os resultados evidenciaram a importância de se refletir sobre os critérios de avaliação clínica utilizados para compreensão da severidade dos casos, sem deixar de considerar os recursos internos dos pacientes relativos à sua capacidade de sofrer, os quais podem ser identificados quando mantida a capacidade de rêverie do profissional. Dr Bawa-Garba, a senior paediatric trainee who had been involved in the care of a child who died shortly after admission to hospital, was convicted of gross negligence manslaughter and subsequently erased from the medical register. We argue that criminalisation of doctors in this way is fraught with ethical tensions at levels of individual blameworthiness, systemic failures, professionalism, patient safety and at the interface of the regulator and doctor. The current response to alleged manslaughter during clinical care is not fit (...) for purpose because of its narrow focus on criminalisation and punishment of individual doctors. The justice system fails to take into account systemic issues in a sufficiently proper and informed way particularly in respect of human factors involved in decision-making. It is easier to convict individual doctors for gross negligence manslaughter than it is to effect legal accountability upon organisations. If educational reflections are used to apportion blame, then this could impact detrimentally on honesty and the duty of candour, with negative implications for patient safety. Regulatory processes should not be deployed without consideration of context. There is an urgent need for a fresh and open evaluation of professional and societal expectations from the regulator that should include positive action as well as those that are proportionately punitive. Justice and patient safety would be served better by more sophisticated contextualisation through an approach that balances accountability in healthcare with failures that can occur within complex systems, and by working to a radical shift towards a just culture. In this paper we discuss the influence of war as a metaphor in the context of the COVID-19 pandemic. After an introduction on the traditional analysis of the war metaphor, we address the social consequences of using this metaphor, a topic that has been widely debated with regard to public communication in the context of COVID-19. We pay particular attention to a theory that many intellectuals have raised: the possibility that the use of the metaphor in this context is harmful (...) to a democratic society because it may lead citizens to accept limited civil liberties and authoritarian policies. After presenting the extensive literature on the use of the war metaphor before and during the COVID-19 pandemic, in the final section of the paper, we analyse experimental evidence of the effects of this metaphor. In the conclusion, we hint at open questions and suggest that the current evidence does not support claims of direct liberticidal influence. (shrink)

In this paper, we have considered a dynamical model of Chlamydia disease with varying total population size, bilinear incidence rate and pulse vaccination strategy. We have defined two positive numbers $$R_{0}$$ R 0 and $$R_{1}$$ R 1. It is proved that there exists an infection-free periodic solution which is globally attractive if $$R_{0} 1.$$ R 1 > 1. The important mathematical findings for the dynamical behaviour of the Chlamydia disease model are also numerically verified using MATLAB. In this work, our aim is to investigate the impact of a non-Kolmogorov predator-prey-subsidy model incorporating nonlinear prey refuge and the effect of fear with Holling type II functional response. The model arises from the study of a biological system involving arctic foxes, lemmings, and seal carcasses. The positivity and asymptotically uniform boundedness of the solutions of the system have been derived. Analytically, we have studied the criteria for the feasibility and stability of different equilibrium points. In addition, we have (...) derived sufficient conditions for the existence of local bifurcations of codimension 1. It is also observed that there is some time lag between the time of perceiving predator signals through vocal cues and the reduction of prey’s birth rate. So, we have analyzed the dynamical behaviour of the delayed predator-prey-subsidy model. Numerical computations have been performed using MATLAB to validate all the analytical findings. Numerically, it has been observed that the predator, prey, and subsidy can always exist at a nonzero subsidy input rate. But, at a high subsidy input rate, the prey population cannot persist and the predator population has a huge growth due to the availability of food sources. (shrink)

International migration is an important issue for many high‐income countries and is accompanied by opportunities as well as challenges. South Asians are the largest minority ethnic group in the United Kingdom, and this diaspora is reflective of the growing diversity of British society. An empirical study was performed to ascertain the faith‐based values, beliefs, views and attitudes of participants in relation to their perception of issues pertaining to end‐of‐life care. Empirical observations from this study, as well as the extant knowledge‐base (...) from the literature, are used to support and contextualise our reflections against a socio‐legal backdrop. We argue for accommodation of faith‐based values of migrants at end‐of‐life within normative structures of receiving countries. We posit the ethically relevant principles of inclusiveness, integration and embedment, for an innovative bioethical framework as a vehicle for accommodating faith‐based values and needs of migrants at end‐of‐life. These tenets work conjunctively, as well as individually, in respect of individual care, enabling processes and procedures, and ultimately for formulating policy and strategy. (shrink)

Significant minority populations confer richness and diversity to British society. Responsive end-of-life care is a universal need that has ascended the public agenda following myriad reports of inadequate provision. Nevertheless, the potential exists for unwitting discrimination when caring for terminally ill patients on the basis of their religion or faith. Recent implementation of the Equality Act 2010, together with the government and professional initiatives, promises to positively impact upon this area of contemporary relevance and concern, although the extent to which (...) facilitative policies can truly enhance patient care will depend upon how these are translated into care at the bedside. The contributions of health professionals will be central in meeting the challenges and seizing the opportunities for meeting the religion and faith interests of patients of South Asian descent. The definition of metabolic syndrome has been, and still is, extremely controversial. My purpose is not to give a solution to the associated debate but to argue that the controversy is at least partially due to the different ‘causal content’ of the various definitions: their theoretical validity and practical utility can be evaluated by reconstructing or making explicit the underlying causal structure. I will therefore propose to distinguish the alternative definitions according to the kinds of causal content they carry: definitions (...) grounded on associations, definitions presupposing a causal model built upon statistical associations, and definitions grounded on underlying mechanisms. I suggest that analysing definitions according to their causal content can be helpful in evaluating alternative definitions of some diseases. I want to show how the controversy over MetS suggests a distinction among three kinds of definitions based on how explicitly they characterise the syndrome in causal terms, and on the type of causality involved. I will call ‘type 1 definitions’ those definitions that are purely associative; ‘type 2 definitions’ the definitions based on statistical associations, plus generic medical and causal knowledge; and ‘type 3 definitions’ the definitions based on mechanisms. These kinds of definitions, although different, can be related to each other. A definition with more specific causal content may be useful in the evaluation of definitions characterised by a lower degree of causal specificity. Moreover, the identification of the type of causality involved is of help to constitute a good criterion for choosing among different definitions of a pathological entity. In section I introduce the controversy about MetS, in section I propose some remarks about medical definitions and their ‘causal import’, and in section I suggest that the different attitudes towards the definition of MetS are relevant to evaluate their explicative power. Cet article examine le rôle joué par la notion d'intelligence dans la poésie de Xénophane et de Parménide. L'auteur soutient que ces deux philosophes, en modifiant les attributs traditionnels du noos humain et divin, répondent aux problèmes posés par le pessimisme gnoséologique de la tradition poétique archaïque, dans laquelle les mortels sont inéluctablement condamnés à l'ignorance. Comme le montre le cas d'Hésiode, même l'inspiration divine ne garantit pas d'acquérir la connaissance, parce que la divinité peut communiquer, à son gré, le (...) vrai ou le faux et que le poète est dépourvu de moyens pour établir la validité de la révélation. Chez Xénophane en revanche, la régularité de l'ordre naturel fondé sur l'immuable noos du dieu offre aux hommes la possibilité effective d'améliorer leur compréhension du monde, tandis que dans le poème de Parménide, la déduction des attributs de l'Être donne aux mortels l'accès à cette vérité qui était auparavant une prérogative exclusive des dieux. The principal aim of this paper is to show how thought experiments are used in the contemporary debate over actual causation and to discuss their role in relation to formal approaches in terms of causal models. I claim that a recourse to thought experiments is not something old fashioned or superseded by abstract models, but it is useful to interpret abstract models themselves and to use our intuitions to judge (...) the results of the model. Recent research on actual causation has stressed the importance of integrating formal models with some notion of normality; I suggest that thought experiments can be useful in eliciting intuitions where normality is not intended in a statistical sense. The first expository part gives a short presentation of the notion of actual causation, summarising some typical problems of counterfactual approaches and how they are treated in causal and structural models. The second part focuses on the problems of model isomorphism and criticises some radical ideas opposing the role of thought experiments, claiming that they may also be of use in evaluating formal models. In this paper, we will analyse the language used by a group of Tourism ESP students on WhatsApp. The data will be analysed from different perspectives: a) the grammatical mistakes made, and b) the students´ degree of adaptation to the most common linguistic features in text messages. The results indicate that these students continue to make some basic grammatical mistakes they should have already overcome. Moreover, many linguistic features of text messages were encountered in their interventions such as punctuation overuse (...) and deletion and overuse of sounds. Finally, they insert many emojis in their messages to express different meanings. (shrink)

This paper questions the dogmatic stance of the domestic courts toward mandatory orders for treatment, arguing that this has the potential to subjugate patients' interests to clinical discretion, and proposing a via media to accommodate the legitimate concerns of all parties.

This collection of essays sheds new light on the relationship between two of the main drivers of intellectual discourse in ancient Greece: the epic tradition and the Sophists. The contributors show how throughout antiquity the epic tradition proved a flexible instrument to navigate new political, cultural, and philosophical contexts. The Sophists, both in the Classical and the Imperial age, continuously reconfigured the value of epic poetry according to the circumstances: using epic myths allowed the Sophists to present themselves as the (...) heirs of traditional education, but at the same time this tradition was reshaped to encapsulate new questions that were central to the Sophists' intellectual agenda. This volume is structured chronologically, encompassing the ancient world from the Classical Age through the first two centuries AD. The first chapters, on the First Sophistic, discuss pivotal works such as Gorgias' Encomium of Helen and Apology of Palamedes, Alcidamas' Odysseus or Against the Treachery of Palamedes, and Antisthenes' pair of speeches Ajax and Odysseus, as well as a range of passages from Plato and other authors. The volume then moves on to discuss some of the major works of literature from the Second Sophistic dealing with the epic tradition. These include Lucian's Judgement of the Goddesses and Dio Chrysostom's orations 11 and 20, as well as Philostratus' Heroicus and Imagines. (shrink)

In 2014, Lord Saatchi launched his ultimately unsuccessful Medical Innovation Bill in the UK. Its laudable aim was to free doctors from the shackles that prevented them from providing responsible innovative treatment. Lord Saatchi’s principal contention was that current law was the unsurmountable barrier that prevented clinicians from delivering innovative treatments to cancer patients when conventional options had failed. This was because doctors feared that they might be sued or tried and convicted of gross negligence manslaughter if they deviated from (...) standard practice. Concerns about fear of the law and potential negative effects on medical practice are not new. Fear of litigation has been suggested as the reason for doctors practising “defensive medicine,” by opting for treatments regarded as “grievance-resistant,” rather than clinically indicated, for example, by ordering diagnostic tests or performing certain procedures, which are not strictly medically necessary. Whilst this claim is plausible and apparently accepted by the courts, there is limited empirical evidence in support of it so far as practitioners in the UK are concerned. In this paper, we report on our empirical research which provides a snapshot of medical opinion to begin to rectify this gap. We ran focus groups of different medical specialties, asking what these medical practitioners thought the barriers to medical innovation to be. We found that fear of the law was not the principal barrier to be lowered, and that the answer was far more multifaceted. (shrink)

La poeta Gloria Dünkler nació en Pucón, Chile, en 1977. Sus padres se dedicaban a la artesanía, la pesca y la música en la región de la Araucanía. Ella estudió Pedagogía en la Universidad de la Frontera y Bibliotecología y Documentación en la Universidad Tecnológica Metropolitana. Actualmente trabaja en la Biblioteca Central de la Facultad de Ciencias de la Universidad de Chile. Su obra editada está compuesta por Quillaco seducido (Edición de autor, 2003), Füchse von Llafenko (Ediciones Tácitas, 2009), Spandau (...) (Ediciones Tácitas, 2012), Yatagán (Ediciones Tácitas, 2015) y Castrato (Ediciones Tácitas, 2020). Además, poemas suyos fueron incluidos en las antologías Mujeres en la poesía chilena actual (Semejanza, 2000), Desde todo el silencio (Los puños de la paloma, 2008), Comer con la mirada (Desde la gente, 2008) y Doce en punto (UNAM, 2012). La obra de Dünkler ha alcanzado amplio reconocimiento en su país, donde obtuvo tres importantes premios: el de la Academia Chilena de Letras (2009), el Premio Municipal de Literatura de Santiago (2012) y el Premio Pablo Neruda de Poesía Joven (2015). Revisión de Tesis Los diarios La Nación y O Estado de São Paulo y sus roles en la legitimación del neoliberalismo en Argentina y en Brasil (1989-1991).

Reflective function is defined as an individual’s ability to understand human behavior in terms of underlying mental states including thoughts, feelings, desires, beliefs, and intentions. More specifically, the capacity of parents to keep their child’s mental states in mind is referred to as parental RF. RF has been linked to adult mental health and parental RF to children’s mental health and development. The gold standard measure of RF is the interview-based Reflective Functioning Scale applied to the Parent Development Interview or (...) Adult Attachment Interview, which while well validated, is time-and labor-intensive to administer. Given the increasing need for reliable, valid, and rapid RF assessment in wide-ranging settings, two alternative measures were considered including the Reflective Function Questionnaire and Parental Reflective Function Questionnaire. We determined the convergent validity of these measures in comparison with the PDI-rated RFS. A sample of mothers and fathers was drawn from a sub-study of the ongoing Alberta Pregnancy Outcomes and Nutrition longitudinal cohort when their children were 42–60 months of age. Pearson correlations and multiple linear regression was conducted, followed by splitting the sample to compute Cohen’s kappas measures of agreement. Two subscales of the PRFQ correlated significantly with the gold standard PDI-rated RFS, providing evidence for convergent validity. As a brief multidimensional measure of parental RF, the PRFQ offers an alternative for measurement of RF in large-scale studies of parental development and child health. (shrink)

Terminally ill patients in 10 states plus Washington, D.C. have the right to take prescribed medications to end their lives (medical aid in dying). But otherwise-eligible patients with neuromuscular disabilities (ALS and other illnesses) are excluded if they are physically unable to “self-administer” the medications without assistance. This exclusion is incompatible with disability rights laws that mandate assistance to provide equal access to health care. This contradiction between aid-in-dying laws and disability rights laws can force patients and clinicians into violating (...) one or the other, potentially creating an underclass of patients denied medical care that is available to those with other (less physically disabling) terminal illnesses. The immediacy of this issue is demonstrated by a lawsuit in Federal court filed in August 2021, requesting assistance in self-administration for terminally ill patients with neuromuscular diseases. This paper discusses the background of this conflict, the ethical issues at the heart of the dilemma, and recommends potential remedies. (shrink)

Firms strive hard to achieve a high degree of efficiency on the production frontier, and it is where they compete to excel with their competitors. It is observed from previous studies that standardizing some policies and developing strategies could bring efficiency in production of manufacturing firms. New approaches to the measurement of productive efficiencies are being developed, and analysis of the subject is growing exponentially, since firms are now more concerned about the productivity frontier, besides others. This paper analyzes the (...) necessary factors that contribute to the productive efficiency of an organization. A simple model of production function is constructed to understand its essence and thus contributes to the development of a new ratio that defines productive efficiency in terms of maximization of the efficiency factor that is a common issue in industrial and manufacturing organizations. This paper is an attempt to understand how social knowledge affects human economic decision making. The paper discusses the nature of social knowledge in today’s context with special reference to how social knowledge influences consumers’ sentiments and their economic decisions. Social networks are being continuously flooded with various kinds of information and disinformation. Some of the information becomes knowledge for social network users who browse various kinds of content that are either entertaining or related to products and marketing. Although reliability (...) remains a critical issue regarding online information from social networks, it, however, provides some knowledge about diverse things, goods, and products being in use in different societies around the world. Some products advertised on social networks like Facebook may affect consumer decisions or provide information about new products. This paper presents a formal discussion on what social knowledge is and how it is originated, and what use it might have for such consumers. The paper designs a simplistic mathematical model for a theoretical understanding of the assumption that has practical implications regarding its utility in society. It is found that social networks generate enough social information that can influence user choice and preferences. Public Libraries (PLs) continue to contribute a great deal to user education in local communities. This paper analyzes the importance of PLs in driving community literacy through promotion of user education for the progressive improvement of the society. The paper stresses the relevance and value of PLs by reassessing the benefits they accrue by analyzing the impact of PLs on community and social education. As indigenous knowledge repositories, PLs play a significant role as community information service providers by driving community (...) literacy. The goal of this paper is to analyze the role played by PLs in driving community education and promoting information literacy in the society. PLs, being storehouses of knowledge, support user education by undertaking active part in community information services. The model helps in explaining and analyzing the efficiency of information search and retrieval process in PLs and how it affects user education and community literacy. The paper also discusses the relevance and value of PLs in this age of digital technology. The findings reinforce the claim that PLs are relevant even in this digital age, and they indeed support user education for the progressive improvement of the society. (shrink)
